To open all all the levels you only have to get one gold, silver, or bronze medal from every level.
It doesn't matter what medal you get. Trust me i tried it before and it works.
To get 99 lives once you've copleted a level you can choose a player [mike or sully] before choosing a player hold L2 and press o x square x2 o x3 and then both players should have 99 lives.
