You can retrieve the Ultima Keyblade by synthesizeing everything on the moogle list. Than you will have the option of synthesizing the ultima keyblade. Trust me it is worth the effort.

It may take a while, in fact, it will. The item list must be completed before you can synthesize for the Ultima Keyblade. You get the items from certain Heartless by killing them and they'll either drop them or not. My trick is equiping the "Lucky Strike" ability to all party members, it helps the heartless drop the items faster. Also using Divine Rose and Lady Luck(The best one) helps out too.

does the keblade change its looks when u get the ultima keyblade?if so does it look cool(i still haven't beaten this one)

To add on what the king said it looks exactly like a sword with some type of gold material swarming around it with a moogle as a key chain.

The oblivion keyblade is at the hollow bastion keyhole room where you fight riku for the second time. The chest for it should be in front of the 7th princess of heart,cell,thing. If you go in front of the keyhole and turn around then you should see it if you don't know where it is.
