The only quest I came across that you need to be ugly was the daedric quest "Namira" your personality has to be 20 or below before doing the quest. Cheap wine was what I used.

What do you mean by Daedric quest? Do you mean you must fight Daedroth (plural for Daedra) or enter Oblivion?

There are Daedric Shrines throughout the land. Usually people are worshipping around a statue. When you have the "right stuff" for the Deadric God you can activate the shrine to do it's mission. I think there is a total of 15 shrines in the game.

I think there are 14 deadric shrines aren't there? I am not at all positive. Also, is it all wine or just cheap wine that makes you ugly.

There are 14 Daedric Shrines, then there is a final (fifteenth) one which can only be activated after completing all other Shrines
Also, all alcohol lowers personality. And anything which lowers personality to below 20 will make you "ugly" enough for the quest
