First might be when I was in a car accident a few years ago. Second when I ended up in a prety messy fight with my then girlfriends father after she was put into hospital because of him. Third would be seeing my mother lying in her hospice bed, in a coma but in so much pain and discomfort, that it was practically palpable.

Wow... Mine was about 3 years ago, me, my mom, dad and sister were returning from Chicago in our Expedition Truck, it began to drizzle and then hail, then it began to pour. We could barley see anything due to the rain and out of the corner of my eye I spotted something darkish, probably gray coming right at us. I though it was a big cloud or something but the rain subsided just enough that I could see a tornado coming from the left I yelled at my mom to hit the gas and she saw another tornado coming behind us, it was brownish/black. My dad began to calm me and my sister down and the truck began to lean to a side and we all were freaking out. My mom strategically got around all of the other cars we all made it home safely. It was terrifying.
One time we were on vacation in Tennessee, hiking in the mountains. We stopped to eat at this river that cut through the trail, and there was another family there. This little kid was was walking on the rocks that broke through the water. Then he saw something that scared him (dunno what it was, probably a rock that looked like a bear or something) and slipped off the rock, busting his head open. Blood was everywhere.
It wasn't terrifying for me but I'm sure he was scared. I just wanted to contribute.
Dunno really, I haven't had too much happen to me, but maybe the time when I sorta slipped in and out of consciousness while in some random place in Denmark because of reasons still unknown to me. Looking back, it wasn't anything big, but at the time I was terrified because from the point of feeling light headed to actually (almost) passing out only took a short time.
Another terrifying moment was my grampa choked on a potato chip and his face began to turn purple because he couldnt breathe. My dad tried the himlec manuver but it didnt work. My grandpa tried drinking water but still nothing and my granpa was literaly dying. So my unkle had to blow in his mouth so he could brethe and he finnaly could breathe again.every one was relived.
The most terrifying moment? I've had a lot of terrifying moments. Once when I was 0-7 years old and my dad would get so mad and start punching in walls and stuff, the most terrifying from that was when he hit my mom in front of me and my brother.(I'm aware I told you guys I didn't have a dad, but I never considered him one). Some other crap my dad did scared me. Umm when I was 8 and went to the top of a lighthouse and it was super windy, the day I was and will always be afraid of heights. When and between now when my mom gets drunk and gets into a rage and literally gets close to hitting me. Yeah, the basic ones I can think of. :P
So the scariest moment would probably be when I was working construction with my dad and I had to get into the rafters (really the trusses because there wasn't anything on the building besides the plywood walls) and they were about... 20 ft in the air i'd say. That was scary. I hate getting up in high places. Oh and once we were working on this house looking over the river, and on the back side it's probably a good 40-50ft drop. I had to be on the wall on the backside and it was pretty dang scary.

First time I was tripping balls on acid probably. I've never been so freaked out in my entire life. Everything red, I thought it was fire. I had a little red dot on my TV, I thought it was fire, I panicked, smashed the TV in to pieces etc. I stayed up all night with a baseball bat. I went out with it, and panicked everytime I saw someone with a red shirt because I thought they were on fire. I've never been through so many panic attacks in one single day. Most horrible thing ever.
