There's a chest in the necrohol (I THINK it's there, might be in the pharos subterra or something) and it has a 1/??? chance to get it, I can't remember the percentage.

The guidebook doesn't say whether you get 2 or not, it just says there's another one. No guarantee at getting 2 though.
