How do you get this title?
Be knifed by someone who has it on.
You get this one by knifing someone who has it on.
How'd these Titles get there?
My only guess is a programmer put it in, then accessed it himself and from there has spread it around.
There is also another one, an emblem:
How do you get it?
No one knows.
It's supposedly available via hacks, but no one knows how to get it legitamitely.

I have STD and Infected, I got them randomly. They're a cool idea. No clue about that emblem though.

That emblem is the 10th Prestige symbol, though most 10th Prestige people I've seen have hacked.
I got Infected, but I haven't received STD yet.

That's the 10th prestige emblem.
The spinning one is the same symbol for 10th prestige, but no one knows how to get it

You can only get the spinning via Hack/Modding.

For the spinning emblem I think you have to do all the challenges, I'm pretty sure Infinity Ward at one point said that.

I have never heard of that happening. EVERY challenge? Come on.
I still go with mod/hack
EDIT: After intense googling, I found that supposedly you can unlock the spinning emblem by completing all of the PRESTIGE challenges. That was the most common answer I found.
