Yeah, just stand over it and hold RB, and you'll pick it up. Hang onto it for a few seconds for it to register that you found the skull. Then you can drop it with Y.

Press RB which is the one above the shoot button. Than drop the skull with Y, after you get the achievement.

Nobody is taking effect the fact that there are different button layouts. John, the three people who answered are correct, but that is for default. If you have a different button layout go to the options, button layout option screen and see which button layout your using to pick it up. And also, another thing is that it won't say 'Hold (Button) to pick up Skull' you will just have to be ontop of it and press the button to pick it up with your button layout.
