one is on the lid of a chest on the second floor.
the second is on the floor with a creak sound when you step on it.
the third is in the room with the piano
and the last is the invisible ink on the note.
I might be wrong about the third one.

They are all right just in a differant order but it doesn't matter but for the answer you gave for the 2nd number you start from the creak and follow the steps to find the number just outside the room but I'll accept, your question.

You fight the lost number and you recieve the Odin Materia and the basement key.

Actually thats not entirely true as I once found out. If you choose not to do anything, and let Sephiroth attack you first, it will trigger Clouds Counter attack materia, winning the battle.
